To ensure that any documents you submit to the Board office make their way to your file, the Board requires that you include a document submittal form as a cover sheet. Complete the form accessible at the link below and submit it each time you mail any documents to the Board office in support of your application. There is no method to submit documentation electronically at this time.

To determine whether specific documents have been delivered to the Board office, you must contact the carrier that you used to deliver your mail for verification of delivery.
