Documents – Altered or Falsified

Question from Online Bar Application:

Have you ever altered or falsified any official or unofficial document or copy thereof (e.g., bar application or examination result letter, recommendation letter, transcript, report, law school application, etc.)?

Instruction from Online Bar Application:

If you answer "Yes" to this question, you must provide a detailed explanation of the circumstances of each incident, including the type of document, the date it was altered or falsified, names of the authorities involved, the address, telephone number and fax number of the individuals/entities involved and the outcome of the action.

Additionally, you must send the Board a copy of the altered or false document if it is available and any written documentation regarding each of these incidents.
