If you have changed your name, please provide the necessary court documents to show proof of the change (i.e. marriage certificate, divorce decree, court order, etc.). If you have not officially changed your name, but have used a different name, please send a letter to the Board office explaining why you are or did use a different name. You must provide to the Board the necessary court documents to show proof of the change (e.g., marriage certificate, divorce decree, court order, etc.). Your name change will not be effective until the Board office receives this documentation. Substantiating documentation is not needed if you are making a typographical correction or if you are truncating your first or middle name to an initial, provided one or the other is in full (e.g., C. Harvey Waldorf, Charles H. Waldorf).
