If you answer "Yes" to this question, enter the military service period information including, the beginning and ending dates of enlistment or commission; the date of discharge; branch of service and serial number. Please provide a separate entry for each period of military service, even if you served within the same branch of service.

Additionally, if you are currently an active duty member of the armed forces, please send the Board office a copy of your most recent Evaluation Report.
