If you answer "Yes" to this question, you must provide a detailed explanation of the current status of the addiction or dependency and how or if it relates to your ability to practice law. Please provide a separate explanation for each addiction or dependency.

Additionally, you may wish to provide a copy of any medical, psychiatric or psychological reports from any of your current or former medical providers that would help the Board understand the nature of your addiction or dependency and its impact on your ability to practice law.
